Registration Process

Step 1

Register the student-athlete on DragonFly

Step 2

Print the NCHSAA Student-Athlete Preparticipation Physical Evaluation Form.

• Parent/Guardian and student-athlete must complete and sign the COVID questionnaire and the History Form (questions 1-32).
• Health care professional must complete the Physical Examination Form and Medical Eligibility Form.

Step 3

Free physicals are available from Blue Ridge Health.  To receive a free physical, parents/guardians must complete the Blue Ridge Health Registration/Permission Form in addition to the COVID questionnaire and History Form on the NCHSAA Physical Evaluation Form.

Step 4

Submit the completed NCHSAA Preparticipation Physical Evaluation Form to the athletic director:

**All students are required to have a physical submitted before they are allowed to try out.
